- MAIL-IN INSTRUCTIONS: hand-write you complete name (no initials), valid e-mail address, street address (P.O. Boxes not accepted), city, state, ZIP code, telephone number, and date of birth (MM/DD/YYYY) on a 3½”x5” card and mail it with sufficient postage to: Miller High Life T-Shirt Merchandise Offer 2016, P.O. Box 410, Southfield, MI 48037.
- After they receive your request they will send you 5 codes. Once you get them, you can enter them here to claim your free t-shirt.
- Important Note: Residents of CA, TX or VA will have to pay a $1 free when you claim your shirt from their website (due to some law I believe). Residents of all other states get it completely free.
